What Pancreatic Cancer Is

According to cancer.org, “Pancreatic cancer is a type of cancer that starts in the pancreas. Pancreatic adenocarcinoma is the most common type of pancreatic cancer. Pancreatic neuroendocrine tumors (NETs) are a less common type…” Your pancreas is that organ that sits behind the stomach. In adults it is typically 6 inches long and less than 2 inches wide.

Pancreatic cancer forms when exocrine cells in the pancreas begin to grow out of control. The majority of the pancreas is made up of these cells which form the exocrine glands and ducts. The exocrine glands make proteins that help you to digest food called pancreatic enzymes. They are released into tiny tubes called ducts. Your pancreas also contains endocrine cells which make hormones like insulin and glucagon, helping to control blood sugar levels.

A vast majority of 95% of cancers of the exocrine pancreas are adenocarcinomas. There are also lesser known exocrine cancers like adenosquamous carcinomas, squamous cell carcinomas, signet ring cell carcinomas, acinar cell carcinomas, undifferentiated carcinomas, and undifferentiated carcinomas with giant cells. There are different growths that can be found in the pancreas, though not all of them are cancer. These include:

  • Serious cystic neoplasms (SCNs)- tumors that have sacs, or cysts, filled with fluid
  • Mucinous cystic pancreatic neoplasms (MCPNs)- slow-growing tumors that have cysts filled with mucin
  • Intraductal papillary mucinous neoplasms (IPMNs)- cystic tumors that grow in the pancreatic ducts
  • Solid pseudopapillary neoplasms (SPNs)- rare, slow-growing tumors usually found in young women

Symptoms, Diagnosis, And Survival Rates

Like with most cancers, it is crucial to catch pancreatic cancer in its early stages. Unfortunately, early stage cancers do not typically cause any symptoms. By the time you experience symptoms, the cancer often grows or spreads outside of the pancreas.

Some of the symptoms include fatigue or physical weakness, jaundice, weight loss and poor appetite, stomach or back pain, nausea and vomiting, gallbladder or liver enlargement, blood clots, and diabetes. Treatment options include surgery, ablation or embolization treatments, radiation therapy, chemotherapy, targeted therapy, immunotherapy, and pain control.

An audit of pancreatic cancer in England and Wales demonstrated that 62% of patients in England and 65% of patients in Wales do not even receive a diagnosis until the cancer has reached stage four. Survival rates are particularly low. 22% of patients do not survive for 30 days after diagnosis in England. In Wales, it is 21% of patients.

Against that reality, researchers are racing to find ways to detect pancreatic cancer earlier, before symptoms become severe. How The Breath Test Works

Scientists at Imperial College London are hopeful that this new breath test will revolutionize treatment of pancreatic cancer. It will be tested for 6,000 patients with an unknown diagnosis across 40 sites in England, Wales, and Scotland. Hopefully the test can be rolled out across GP surgeries in the next five years, all in the hopes of early detection.

According to an article from independent.co.uk, “The test is used to detect a combination of ‘volatile organic compounds’ present in the breath. Thousands of these compounds travel around the bloodstream and are filtered out when the blood reaches the lungs and then expelled out with breath, the charity said. It said these changes are evident even when the cancer is in its earliest stage of disease. Isolating unique combinations of these compounds can pinpoint whether or not a person has pancreatic cancer, with results available for GPs in just three days.”

Funding And Expert Reactions

Pancreatic Cancer UK is providing £1.1 million to fund the study. The chief executive Diana Jupp said, “The breath test has the potential to revolutionise the early detection of pancreatic cancer. It is, undoubtedly, the most significant step toward a lifesaving breakthrough in 50 years. While more years of development are still needed before we can put this exciting new technology into the hands of GPs across the country, thousands of patients with an unknown diagnosis will now help refine it in the real-world. This is the first pancreatic cancer breath test to ever reach a national clinical trial of this scale. That in itself makes this a moment of real, tangible hope. For decades the deadliest common cancer has been seen as too great a challenge to solve but we are determined to keep pushing the boundaries of what’s thought possible.”

The leader of the project and head of the Department of Surgery and Cancer at Imperial College London, Professor George Hanna said, “If our findings from the initial phase of the breath test study can be validated in a population of patients with an unknown diagnosis, it has huge potential to influence clinical practice and pancreatic cancer referral pathways. The funding announced today means we can now move quickly to that patient validation study stage, which is a very exciting next step for us. We look forward to seeing how the test performs in this group of patients with suspected cancer.”
